Clemencia Rasquinha, MD, CMD
Dr. Rasquinha practices the true art of Geriatric Medicine. She loves to share her passion for geriatrics and aging by speaking on senior health topics to various organizations. Dr. Rasquinha graduated from Bangalore University in India, served her residency and internship at Bergen Pines County Hospital in Paramus, NJ, and completed fellowships in clinical nutrition, hospital leadership and is a board certified Geriatrician. She carries the title Certified Medical Director from the board of post-acute and long-term care medicine. She has served as medical director for home care, hospice, rehabilitation medicine and long-term care. She was instrumental in creating Clinical Pearls and Guidelines in Geriatric Medicine. Dr. Rasquinha is married to husband, Cecil Asher; she has two adult children and is a dog enthusiast and counts a Portuguese water-dog and a Puggle among her family members. She enjoys cooking, walking outdoors and always strives to keep her mind and body active.

William B. Ogden, MD
William received his undergraduate degree from Northwestern University in Evanston, IL. He went on to complete their Honors Program in Medical Education with highest honors at Northwestern University School of Medicine. His internship and residency were done at the University of Minnesota. He is American Board of Internal Medicine (ABIM) certified. His career has included owning a successful long-standing private practice in St. Paul that was sold and is now part of Allina Health. William enjoys forming strong relationships with his patients and working to improve their health. He likes the intellectual stimulation that often seems like detective work. He taught Internal Medicine to Family Medicine Residents every year of his nearly 43 year practice, winning innumerable Teacher of the Year awards. After he retired in January 2019, Dr. Ogden realized the hardest part of retirement actually was loss of patient contact and care. He joined TCP September 2019. When not in the office, William enjoys gardening, photography, and an occasional road trip. He is also fond of spending time with family, including his grandchildren. He and his wife are animal lovers, as his wife breeds and shows dogs.

Hannah Reimen, DO
Dr. Reimen brings a wealth of experience and knowledge in geriatrics to our team. She received her Bachelor’s degree from Middlebury College in Vermont and her Medical Degree from the University of New England – College of Osteopathic Medicine in Maine where she also received The Humanism in Aging award and was president of The American Geriatrics Society. During her residency, Hannah was also elected Chief Resident. She has worked in a variety of different settings including nursing homes, transitional care, and inpatient acute care. Hannah strives to build each patient relationship on the foundation of trust and ensuring their goals of care are being met. When not at work, she enjoys exploring the Twin Cities, riding her bike, walking her 3 dogs, and spending time with her family.

Isaac Adediran, MD

Dr. Adediran brings a wealth of experience and knowledge to our team at Twin Cities Physicians. Completing his residency at Beaumont Hospital, his skills in both hospital and clinic settings allow him to better care for his patients, their families, and really focus on what brings them the best quality of life. Dr. Adediran believes the first step when caring for someone is compassion. Being able to connect with those outside of their medical needs has helped him care for his patients as a whole and brought on better outcomes. When not seeing his patients, you can find Dr. Adediran playing soccer, watching sports, traveling, and spending time with his family and friends.

Steven Atkinson, PAC, MS
Steven Atkinson is a Board Certified Physician Assistant specializing in Geriatric Internal Medicine. He has worked with Dr. Rasquinha for more than 15 years. He is adjunct faculty at the University of Utah and has done so since 1994; he has been involved in medicine since 1988. He is dynamic speaker and lectures both nationally and internationally. He is a published author in the area of Geriatric Pharmacology and has served on several boards related to geriatric medicine and care. He is an avid outdoors enthusiast and enjoys running in almost every destination he travels to. He enjoys working with the elderly and considers it his “passion” in life. His positive personality and energy have earned him several local awards in the arena of Geriatric Adult Medicine.

Heather Arechigo, MSN, AGNP-C
Heather Arechigo is a Nurse Practitioner who specializes in Adult-Gerontology. Prior to becoming a Nurse Practitioner, Heather worked in an Intensive Care Unit specializing in adult critical care & neurologic crisis. She holds a Master’s of Science in Nursing from Kaplan University where she graduated with honors & is still a member of the S.A.L.U.T.E honor society that recognizes academic excellence and leadership qualities. She also holds a Bachelor of Science in Nursing degree from the College of Saint Catherine in St. Paul, MN. Heather was a student teacher of anatomy and physiology during her undergraduate nursing program. In addition to her professional career, Heather is a Combat Veteran having served in Operation Iraqi Freedom as a member of the US Army from 2001-2009. When not at work, she spends time with her husband and two beautiful daughters. Heather values family time, learning to speak Spanish as a family and active hobbies like snowboarding & running.

Jeannine Phillips, PA-C, MS
Jeannine, “Jeannie” to her patients, is a second career PA. She graduated from Marquette University with a Masters in Physician Assistant Studies in 2007 and began her PA journey in Mankato, MN in urgent care and family practice settings for four years. She then transferred to Sibley Medical Center/Ridgeview to develop a blended nursing home and clinic practice.  It was during this interval that she realized that her true calling was to care for the geriatric population while educating and supporting their family members. Jeannie joined Twin Cities Physicians in March 2016. Her previous career, as a teacher, brought her overseas to England and Thailand. She received her Bachelors in Music education, Chemistry and Physical Science Education from Saint Olaf College in 1987.  When not at work, Jeannie loves directing her church brass group, traveling, working on home projects, gardening, landscaping, enjoying the outdoors, and riding her motorcycle.

Gayle Bauer, AGNP-C
Gayle is an Adult-Gerontological Primary Care Practitioner certified by the AANP and the ANCC. She graduated with her Bachelor of Science in Nursing from St. Olaf College and attained her Masters of Science in Nursing from Kaplan University, holding many positions in healthcare along the way. For the past 25 years, she has spent her career as an Assistant Director of Nursing at the Northfield Hospital Long Term Care Center. Gayle believes that a person is never too old to learn and enjoys facilitating her patients and their families with creating a care plan that is more individualized for their needs. When not at work, Gayle enjoys the outdoors, and you can probably find her helping her husband at their farm, playing golf, or enjoying a bonfire.

Haley Luckew, APRN, CNP
Haley is a Board Certified Nurse Practitioner who strives to create an individualistic care plan for each of her patients. She received her Bachelor of Science in Nursing from Winona State University and completed her Doctor of Nursing Practice (DNP) at the University of Minnesota. Haley has worked in a variety of settings including home care, hospital, and a cardiac care unit. She loves creating a bond with her patients and their families to ensure each patient can manage their health and medications in the most positive way possible. When not at work, Haley enjoys spending time with her family, reading, and enjoying the Minnesota weather.

Emma Dolphin, PA-C, MPAS
Emma received her Bachelor of Science in Biology, with a minor in Chinese, from Butler University in Indianapolis, Indiana. She obtained her Master of Physician Assistant Studies from Gardner-Webb University in North Carolina. She has worked in long term care settings, as well as done research in Alzheimer’s and dementia care. Being of service to older adults remains one of her biggest passions as a provider, noting that “they paved the way for us!” Emma likes the career flexibility of being a medical professional. When not at work, she enjoys running, hiking, training her new truffle-hunting/agility dog, Mia, and spending time with family. She is also active as a volunteer in her church and community.

Angela Score, DNP, APRN, AGNP-C
Angela received her Bachelor of Science in Nursing from Illinois Wesleyan University. She obtained her Doctor of Nursing Practice from the University of Minnesota, having completed their Adult-Gerontology Primary Care Nurse Practitioner (AGNP) program. Her professional experience has included time on a medical surgical stroke unit, as well as inpatient oncology work. Angela enjoys meeting new patients, learning about their life, and being able to work with them to make sure they can live a healthy, enjoyable life. When not in the office she loves to travel with her husband, read a good book, and enjoy a cup of coffee. She stays active by trail running, working out, and taking her dog, Kirby, to the dog park.

Becky Schaefer, RN, PHN, APRN, CNP
Becky received her Bachelor of Science in Nursing from Minnesota State University, Mankato, and her Master of Science in Nursing from the University of North Dakota, Grand Forks. She completed an internship at St. Joseph’s Hospital in St. Paul in their oncology unit. Her previous experience includes work as a nurse practitioner in a long-term care setting, as well as critical care work at St. John’s Hospital in their intensive care unit and emergency room. She enjoys making meaningful connections with her patients and their families. Her goal is to work with patients and staff to help patients reach their desired goals. When not in the office, Becky enjoys going to concerts, listening to music, as well as a variety of outdoor activities. She also loves spending time with family, cooking, and eating great food.

Jessicah Gabrielson, APRN, FNP,  CNP
Jessicah Gabrielson received her Bachelor of Science in Nursing from the College of St. Scholastica in Duluth, MN and went on to receive her Master of Science in Nursing from Saint Luke’s College of Health Sciences in Kansas City, MO. She has experience working in several settings including an emergency room and an intensive care unit (ICU). Throughout her career as a nurse and nurse practitioner, Jessicah finds that forming relationships with patients and helping them lead healthier lives is the most fulfilling part of her role.
Outside of her work, Jessicah enjoys reading, travelling, and being outdoors with her husband and children.

Malinda Lynch, DNP, FNP
Malinda received her Bachelor of Science in Nursing from Walden University in Minneapolis, MN. She then pursued her Doctor of Nursing Practice at Metropolitan State University in St. Paul, MN. She is certified as a Family Nurse Practitioner through the American Association of Nurse Practitioners. Malinda has provided nursing care to clients in a variety of settings—including independent and assisted living senior communities, as well as private homes—through her work for a prominent Twin Cities’ home care and hospice agency. She enjoys improving the quality of life for others through a holistic approach, in often dynamic and challenging environments. When not in the office, Malinda enjoys fishing, hiking in nature, and spending time with her family.

Gina Schmit, PA-C, MPAS

Gina received her Master of Physician Assistant Studies from Concordia University Wisconsin in Mequon, WI. She has experience working in a variety of settings including primary care practices, outpatient clinics, and long-term-care communities.  Gina likes having the knowledge to safely treat and provide better care for her patients. She finds that building relationships with her patients helps improve communication and continuity of care to achieve the highest possible quality of life. Outside of work, Gina loves spending time with her family and friends, staying active, reading books and playing board games. She’s got a soft spot for a good puzzle and enjoys travelling, too!

Angela Bradford, MSN, AGNP-C

Angela is a Board-Certified Adult Gerontology Nurse Practitioner who enjoys creating great connections with those she cares for. She graduated with her bachelor’s degree from The College St. Scholastica and attained her Master of Science in Nursing from St. Catherine University. Angela’s experience in the operating room, PACU, med-surg, home care, and nursing home setting across the country gives her a breadth of knowledge that helps her follow her patients wherever they call home. When not at work, Angela enjoys spending time working in her garden, baking, and working on projects with her husband.

Jessica Galbrecht, DNP, CNP, PHN

Jessica received her Bachelor of Science in Nursing and Nurse Educator Master of Science in Nursing from Minnesota State University – Moorhead as well as her Adult Gerontology Primary Care Nurse Practitioner (AGCNP) Master of Science in Nursing and Doctor of Nursing Practice from the University of North Dakota. She is certified with the American Academy of Nurse Practitioners and has held positions in a myriad of healthcare settings including leadership and education roles. She understands the importance of individualized care and takes a patient centered approach to give the best care to her patients and their families. In her free time, Jessica enjoys spending time with her husband and daughter, camping and spending time at the lake, traveling, and staying active.

Sarah Marti, PA-C, MSPAS

Sarah received her Bachelor of Arts in Music from St. Olaf College and earned her Master of Physician Assistant Studies from Augsburg University. Working in memory care, hospital, hospice, and laboratory settings has helped lay a great groundwork of skills that makes Sarah a valuable addition to our team. She enjoys helping her patients figure out what quality of life means to them and finding ways to help reach their goals. When not working and seeing her patients, you can find Sarah spending time with her dog, baking, or playing the piano or violin!

Veronicah Chweya, AGCNP

Veronicah is a Board-Certified Adult Gerontology Nurse Practitioner who strives to make a difference in her patient’s lives by providing high quality care and focusing on their overall health and well-being. Previously working as a Registered Nurse in hospital, TCU, long-term care, and home care settings, her experience helps her better connect with her patients and their families. Veronicah’s passion in lending a helping hand to those needing medical attention has always assisted her in developing closer relationships with those around her. When not working with her patients, you can find Veronicah spending her free time with her family, reading, and traveling.

Cody Tucholke, PA-C, MPAS

Cody earned his Master of Physician Assistant Studies from St. Catherine University and is a credentialed Physician Assistant by the National Commission on Certification of Physician Assistants. Throughout his undergraduate education, he worked with ACR Homes caring for adults with disabilities. This setting gave Cody a chance to better connect with his patients and collaborate in a team-based setting. He enjoys the opportunity to provide his patients with evidence-based, personable, and values-based care and believes that making a connection is an important part in providing that care. In his free time, you can find Cody exercising, spending time with his wife and newborn son, or participating in his church community.

Simon Mazza, PA

Simon received his Bachelor of Science in Biology from the University of St. Thomas and earned his Master of Physician Assistant studies from Augsburg University. Previously working in Family Medicine and Clinic settings, he has been able to connect with patients at many different stages of their healthcare journey and assist them in getting the right care they deserve. He enjoys being able to connect with each patient on an individual level and collaborate to create a care plan that helps them achieve their goals. In his free time, you can find Simon spending time with his family and friends, exploring new hobbies and traveling.

Katelyn Elhard, PA-C, MPAS

Katelyn received her bachelor’s degree from Minnesota State University, Mankato and obtained her Master of Physician Assistant Studies from St. Catherine University. Working in group home and medical/surgical oncology units as a CNA and direct support professional have given her skills that allow her to better connect with her patients and deliver the highest quality of care possible. Katelyn enjoys the opportunity to help promote health through education. Being able to form relationships with her patients allows her to focus on their values and reach the quality of life they desire. When not seeing her patients, you can find Katelyn being active outdoors or spending time with her husband, friends, and their two dogs.

Leah Li, PA-C

Leah graduated with her Bachelor of Science in Biology from the University of St. Thomas and earned her Master of Physician Assistant Studies from Le Moyne College in Syracuse, NY. Previously working in outpatient, inpatient, OR and group home settings, Leah has a wide array of skills that allow her to build better relationships with her patients. Being a person that her patients and their families can rely on is one of her favorite parts of her role. When not seeing her patients, you can find Leah cooking, traveling, or enjoying time with her friends and family.

M. Irene Wangechi, NP

Irene is a Nurse Practitioner with Twin Cities Physicians. She received her education from Maryville University of St. Louis. She has experience working in medical surgical units, oncology, and hospice with a focus on geriatrics. Irene enjoys making a positive difference in the lives of her patients. Being someone that her patients can connect with gives her the opportunity to provide them with quality care and help meet their healthcare goals. Outside of work, Irene can be found exercising, especially going for a jog in her favorite local spot.

Rebecca Gibson, APRN, AGPCNP-C
Rebecca is a board-certified Adult-Gerontology Primary Care Nurse Practitioner and received her education through the University of Minnesota School of Nursing. Working primarily in geriatric and skilled nursing facility settings for over 20 years has given Rebecca the skills and knowledge to truly connect with her patients so they can be heard and have the best quality of life. Her role allows her to share her passion for caring for her patients, their families and staff members to meet everyone’s needs. When not in the office or seeing her patients, you can find Rebecca enjoying her time with her dog at the park, going to the movies or spending time with family and friends!

Mara Trudgeon, MSN, APRN, AGNP-C
Mara received her Bachelor of Science in Nursing from the University of Minnesota and went on to receive her Master of Science in Nursing from St. Catherine University. Currently, she is continuing her education through a Doctorate of Nursing program at St. Catherine University. She has previously worked with the University of Minnesota Medical Center as a registered nurse on a medical/surgical floor focusing on specialty surgeries and complex medical conditions as well as a clinical instructor for nursing assistant students as St. Catherine University. She enjoys serving her patients and their families and watching them achieve their
healthcare goals. Being able to work with people from all walks of life allows for many learning experiences that help her better serve her patients. Outside of her role with Twin Cities Physicians, you
can find Mara going on walks with her dogs, Buster & Scooter, practicing yoga and cross-stitching.

Nic Southerton, AGNP

Nic received his Bachelor of Science in Nursing from Rasmussen University and earned his Master of Science in Nursing from Walden University after serving as a Corpsman in the Navy where he found his passion for medicine. Being able to learn in outpatient surgery, ENT & plastic surgery, medical/surgical, infusion clinic and combat medic settings has given Nic a very diverse skillset that has helped him in various situations. He has a great passion for providing education to his patients and their families and always wants to make sure that he is available to assist them in enhancing their overall wellbeing. When not seeing his patients, you can find Nic traveling to new places, enjoying the outdoors any time of year, or spending time with his family, friends and pets.

Kate Forsell, DNP, APRN, FNP-C

Kate Forsell graduated with her Bachelor of Science in Nursing as well as Public Health Nursing from Augsburg University. After completing her undergrad, she continued on to earn her Doctor of Nursing Practice as a board-certified Family Nurse Practitioner. Prior to working in her role as a primary care provider, Kate worked in a multitude of settings including geriatric home care, urgent care settings, and TCU settings all as a registered nurse or clinical supervisor role. Kate particularly enjoys working with the geriatric population because of the relationships she is able to form with her patients. Being able to watch their quality of life improve based off of her care and interactions with her is one of the best parts of her job. When not seeing her patients, you can find Kate hiking or reading a good novel or at the stables with her husband and three children spending time with their horse.

Ashley Weis, AGNP

Ashley received her Bachelor of Science in Nursing from Crown College and went on to earn her Master of Science in Nursing from Maryville University in St. Louis, Missouri. Prior to working in primary care as a nurse practitioner, Ashley worked as a registered nurse for 5 years in medical/surgical, cardiac care, clinic as well as travel nursing settings. Being able to make meaningful connections with her patients and their family members regardless of her role is one of her favorite parts of working in the healthcare field. She enjoys being able to be an advocate for her patients and believes that finding out more about each person’s mind, body and spirit can help facilitate better care and a more purposeful relationship. When not seeing her patients, you can find Ashley hiking, backpacking or camping outdoors and spending quality time with her friends, family and dog, Bailey!

Thomas Ondara, FNP-C

Thomas Ondara completed both his Bachelor of Science in Nursing and his Master of Science in Nursing from the University of North Dakota in Grand Forks. Prior to joining the team at Twin Cities Physicians, Thomas gained experience in many different areas of health care, including: Long Term Care, Transitional Care Units, Internal Medicine and more. Being able to make a meaningful impact in his patients’ lives is at the forefront of his care. Whether it be getting them the resources and access to care they need or providing a regular visit to discuss their healthcare concerns, he always looks for ways to positively impact the lives of others. When not seeing his patients, you can find Thomas staying active by playing soccer & running marathons, traveling, and writing stories & medical journals.

Jessica Spurrier, MSN, APRN, FNP-C

Jessica Spurrier earned both her Bachelor of Science in Nursing as well as her Master of Science in Nursing from the University of North Dakota in Grand Forks. Prior to becoming a board-certified Family Nurse Practitioner, Jessica practiced in a multitude of settings, including: cardiovascular surgery, ICU, med/surg, telemetry, PACU and even travel nursing. Having previous experience in these settings has given Jessica great experience in working with complex cases and on diverse teams. Aside from providing hands-on cares, Jessica truly enjoys being able to make a connection with her patients & their families. Knowing who they are and what they care about has allowed her to add value to her visits as she can better personalize the care for each individual. When not providing care for her patients, you can find Jessica staying active & being outdoors, traveling, and spending quality time with her friends, family & pets!

Hannah Hollenhorst, MSN, APRN, AGNP-C

Hannah received her Bachelor of Science from the University of North Dakota and her Master of Science in Nursing from St. Catherine University specializing in geriatrics, primary care and chronic disease management. Before her role as a nurse practitioner, Hannah gained experience and knowledge in cardiology, gerontology, primary care, internal medicine and inpatient acute care cardiology. Being well-versed in these settings has allowed her to make a better connection with her patients and ensures that she can create individualized care plans they deserve. As a member of the American Association of Nurse Practitioners and a Certified Adult-Gerontology Nurse Practitioner, Hannah spends a lot of time on professional development and staying on top of new advancements and trends in the healthcare field. When not seeing her patients, you can find Hannah spending time volunteering in local community health programs, exercising with her dog and spending quality time with her family and friends.

Jennifer Gertgen, APRN, CNP

Jennifer brings a wealth of knowledge and experience to Twin Cities Physicians. She earned her Master of Science in Nursing from Maryville University in St. Louis, Missouri and has held a wide variety of positions in different settings along the way, including: primary care, orthopedics, assisted livings, hospitals, hospice and even large healthcare systems. Throughout her tenure in the healthcare field, Jennifer has found it most enjoyable to build relationships with those she serves. Being able to provide continuity of care to her patients brings her peace of mind knowing that she is providing comfort to them, their families and community. When not seeing her patients, you can find Jennifer spending her time outdoors! She enjoys camping, hunting and spending time with her two granddaughters.

Dawn Bos, FNP-BC, PMHNP-BC, PhD

Dawn received her Master of Science in Nursing from South Dakota State University as well as her Post Master Certificate as a Psychiatric Mental Health Nurse Practitioner from the University of Nebraska Medical Center. She started her career providing specialized mental health care, but since has practiced exclusively in primary care working in substance use treatment facilities, nursing homes, detox and mental health facilities in more rural settings. Dawn sees the importance of providing personalized care to those where resources are limited. She enjoys working with all those involved in her patients care & takes pride in being  able to develop collaborative care plans to improve quality of life. When not seeing her patients, you can find Dawn enjoying a variety of things! You can find her outdoors and staying active as well as relaxing with her grown children and grandchildren. She also enjoys volunteering and is actively learning Spanish to work with people of Latino heritage.
